    Global Stability of Humoral Immunity HIV Infection Models with Chronically Infected Cells and Discrete Delays

    We study the global stability of three HIV infection models with humoral immune response. We consider two types of infected cells: the first type is the short-lived infected cells and the second one is the long-lived chronically infected cells. In the three HIV infection models, we modeled the incidence rate by bilinear, saturation, and general forms. The models take into account two types of discrete-time delays to describe the time between the virus entering into an uninfected CD4+ T cell and the emission of new active viruses. The existence and stability of all equilibria are completely established by two bifurcation parameters, R0 and R1. The global asymptotic stability of the steady states has been proven using Lyapunov method. In case of the general incidence rate, we have presented a set of sufficient conditions which guarantee the global stability of model. We have presented an example and performed numerical simulations to confirm our theoretical results

    The impact of the Lombard effect on audio and visual speech recognition systems

    When producing speech in noisy backgrounds talkers reflexively adapt their speaking style in ways that increase speech-in-noise intelligibility. This adaptation, known as the Lombard effect, is likely to have an adverse effect on the performance of automatic speech recognition systems that have not been designed to anticipate it. However, previous studies of this impact have used very small amounts of data and recognition systems that lack modern adaptation strategies. This paper aims to rectify this by using a new audio-visual Lombard corpus containing speech from 54 different speakers – significantly larger than any previously available – and modern state-of-the-art speech recognition techniques. The paper is organised as three speech-in-noise recognition studies. The first examines the case in which a system is presented with Lombard speech having been exclusively trained on normal speech. It was found that the Lombard mismatch caused a significant decrease in performance even if the level of the Lombard speech was normalised to match the level of normal speech. However, the size of the mismatch was highly speaker-dependent thus explaining conflicting results presented in previous smaller studies. The second study compares systems trained in matched conditions (i.e., training and testing with the same speaking style). Here the Lombard speech affords a large increase in recognition performance. Part of this is due to the greater energy leading to a reduction in noise masking, but performance improvements persist even after the effect of signal-to-noise level difference is compensated. An analysis across speakers shows that the Lombard speech energy is spectro-temporally distributed in a way that reduces energetic masking, and this reduction in masking is associated with an increase in recognition performance. The final study repeats the first two using a recognition system training on visual speech. In the visual domain, performance differences are not confounded by differences in noise masking. It was found that in matched-conditions Lombard speech supports better recognition performance than normal speech. The benefit was consistently present across all speakers but to a varying degree. Surprisingly, the Lombard benefit was observed to a small degree even when training on mismatched non-Lombard visual speech, i.e., the increased clarity of the Lombard speech outweighed the impact of the mismatch. The paper presents two generally applicable conclusions: i) systems that are designed to operate in noise will benefit from being trained on well-matched Lombard speech data, ii) the results of speech recognition evaluations that employ artificial speech and noise mixing need to be treated with caution: they are overly-optimistic to the extent that they ignore a significant source of mismatch but at the same time overly-pessimistic in that they do not anticipate the potential increased intelligibility of the Lombard speaking style

    Traumatic bilateral dissection of cervical internal carotid artery in the wake of a car accident: A case report

    Background Bilateral carotid artery dissection secondary to severe trauma is rare and can be potentially life -threatening if not diagnosed and treated properly. Case Presentation We report a 29-year-old female who was admitted to the emergency department after a car accident. The patient was conscious at the time of admission and presented with an initial Glasgow Coma Scale (GCS) of 15 presenting normal vital signs. The patient developed motor dysphasia with right upper limb paresis a few hours after the admission. Magnetic resonance imaging (MRI) revealed a bilateral cervical internal carotid artery (ICA) occlusion in addition to left frontal lobe infarct in a subacute phase. Medical management was successful and the patient was discharged from the hospital two weeks after the admission. Discussion Noninvasive vascular imagining modalities are merging as the gold standard in the early detection of carotid artery dissection (CAD). Typical pathognomonic findings on MRI include double lumen and intimal flap. The management with systemic anticoagulation or antiplatelet therapy is aimed to prevent the development of ischemic stroke. In case of medical therapy being ineffective or in case of complication or any disorders suffered by a patient, endovascular treatment is performed. Conclusion With early detection and proper management, traumatic dissection of cervical carotid artery can have a benign outcome. As for the current patient, medical treatment with anticoagulation was sufficient and surgical management was therefore not required. Improvement in the patients’ speech was observed; nevertheless the continuation of speech therapy was indicated

    Compilation of an Arabic Children’s Corpus

    Inspired by the Oxford Children's Corpus, we have developed a prototype corpus of Arabic texts written and/or selected for children. Our Arabic Children's Corpus of 2950 documents and nearly 2 million words has been collected manually from the web during a 3-month project. It is of high quality, and contains a range of different children's genres based on sources located, including classic tales from The Arabian Nights, and popular fictional characters such as Goha. We anticipate that the current and subsequent versions of our corpus will lead to interesting studies in text classification, language use, and ideology in children's texts

    The Need for Emergency Management Models

    Emergency agencies can use emergency management models to enable them to better prepare for and respond to emergencies. This qualitative study aims to undertake a critical examination of emergency management models by thematic analysis to determine their significance to emergency management. A review and analysis of the existing literature were used in the study. The models were studied to explore their role in emergency management and to identify any significant constraints or challenges which could limit the ability of the emergency management model to carry out appropriate actions. The study found that such models are indispensable because they simplify and improve emergency management. Additionally, they may support planners, managers, and practitioners in reaching proper decisions, making them a valuable and necessary decision-making support tool. The study also showed that each model has an advantage that distinguishes it from the other models. Consequently, a comprehensive emergency management model should be designed to suit all cases and circumstances. The findings also confirmed the doubts raised about the limitations and concerns associated with the models. Concerns included future events’ unpredictability, the models’ prescriptive nature, the event’s cultural context, and the impact on businesses. The findings also indicated that certain planners, managers, and practitioners had a limited understanding of the use of models in emergency management. As such, they appear to have overlooked the use of models while dealing with emergencies. Hence, the study recommends that the models should be employed in all emergency management activities. The study also recommends that the findings are utilized as a basis for further research into the potential use of emergency management models

    Blockchain Empowered Federated Learning Ecosystem for Securing Consumer IoT Features Analysis

    Resource constraint Consumer Internet of Things (CIoT) is controlled through gateway devices (e.g., smartphones, computers, etc.) that are connected to Mobile Edge Computing (MEC) servers or cloud regulated by a third party. Recently Machine Learning (ML) has been widely used in automation, consumer behavior analysis, device quality upgradation, etc. Typical ML predicts by analyzing customers’ raw data in a centralized system which raises the security and privacy issues such as data leakage, privacy violation, single point of failure, etc. To overcome the problems, Federated Learning (FL) developed an initial solution to ensure services without sharing personal data. In FL, a centralized aggregator collaborates and makes an average for a global model used for the next round of training. However, the centralized aggregator raised the same issues, such as a single point of control leaking the updated model and interrupting the entire process. Additionally, research claims data can be retrieved from model parameters. Beyond that, since the Gateway (GW) device has full access to the raw data, it can also threaten the entire ecosystem. This research contributes a blockchain-controlled, edge intelligence federated learning framework for a distributed learning platform for CIoT. The federated learning platform allows collaborative learning with users’ shared data, and the blockchain network replaces the centralized aggregator and ensures secure participation of gateway devices in the ecosystem. Furthermore, blockchain is trustless, immutable, and anonymous, encouraging CIoT end users to participate. We evaluated the framework and federated learning outcomes using the well-known Stanford Cars dataset. Experimental results prove the effectiveness of the proposed framework

    Safety of the Deferral of Coronary Revascularization on the Basis of Instantaneous Wave-Free Ratio and Fractional Flow Reserve Measurements in Stable Coronary Artery Disease and Acute Coronary Syndromes.

    OBJECTIVES: The aim of this study was to investigate the clinical outcomes of patients deferred from coronary revascularization on the basis of instantaneous wave-free ratio (iFR) or fractional flow reserve (FFR) measurements in stable angina pectoris (SAP) and acute coronary syndromes (ACS). BACKGROUND: Assessment of coronary stenosis severity with pressure guidewires is recommended to determine the need for myocardial revascularization. METHODS: The safety of deferral of coronary revascularization in the pooled per-protocol population (n = 4,486) of the DEFINE-FLAIR (Functional Lesion Assessment of Intermediate Stenosis to Guide Revascularisation) and iFR-SWEDEHEART (Instantaneous Wave-Free Ratio Versus Fractional Flow Reserve in Patients With Stable Angina Pectoris or Acute Coronary Syndrome) randomized clinical trials was investigated. Patients were stratified according to revascularization decision making on the basis of iFR or FFR and to clinical presentation (SAP or ACS). The primary endpoint was major adverse cardiac events (MACE), defined as the composite of all-cause death, nonfatal myocardial infarction, or unplanned revascularization at 1 year. RESULTS: Coronary revascularization was deferred in 2,130 patients. Deferral was performed in 1,117 patients (50%) in the iFR group and 1,013 patients (45%) in the FFR group (p < 0.01). At 1 year, the MACE rate in the deferred population was similar between the iFR and FFR groups (4.12% vs. 4.05%; fully adjusted hazard ratio: 1.13; 95% confidence interval: 0.72 to 1.79; p = 0.60). A clinical presentation with ACS was associated with a higher MACE rate compared with SAP in deferred patients (5.91% vs. 3.64% in ACS and SAP, respectively; fully adjusted hazard ratio: 0.61 in favor of SAP; 95% confidence interval: 0.38 to 0.99; p = 0.04). CONCLUSIONS: Overall, deferral of revascularization is equally safe with both iFR and FFR, with a low MACE rate of about 4%. Lesions were more frequently deferred when iFR was used to assess physiological significance.     Diuretic activity of carob (Ceratonia silique L.) Honey: comparison with furosemide

    Background: Honey has wide range of biological activities. It has effect on renal function, and urinary nitric oxide and prostaglandins level. The present study was conducted to evaluate diuretic potential of carob honey, collected from Morocco, in normal rats and the results were compared with use of furosemide.Materials and methods: Adult male Wister rats weighing between 230 and 278 g were used. The animals were divided into three groups; group 1 received oral administration of distilled water (10 ml/kg BW), and served as control group, group 2 received oral administration of furosemide (10 mg/kg BW), and group 3 was treated with oral administration of carob honey (100 mg/kg BW). Urine volume, and urine and plasma sodium and potassium were measured after single dose of the interventions and after daily administrations of the interventions.Results: After the single dose of carob honey, urine output was significantly increased at all time intervals (1-6 hrs and at 24 hrs after administration). The daily dose of carob honey for nine days significantly increased urine volume as compared to control group. Carob honey increased urinary levels of sodium and potassium, and did not cause hypokalemia, while furosemide increased urinary sodium and potassium and caused hypokalemia.Conclusion: Carob honey has diuretic, natriuretic and kaliuretic activity without side effects of hypokalemia that was observed with use of furosemide.Key words: Carob honey, furosemide, diuresis, sodium, potassium, creatinine clearanc
